Single Implant-Supported Bridge: Immediate Aesthetic Recovery for Anterior Tooth Loss & Complex Bone Deficits

This aesthetic immediate restoration case was led by Dr. Tang Caomin, Associate Chief Physician at Chengdu Anyu Dental Implant Hospital and an 8-year Ph.D. graduate from the West China School of Stomatology, Sichuan University.

The patient presented with significant aesthetic distress and severe chewing difficulties following a trauma-induced loss of Tooth #2 (lateral incisor) years prior. Due to the visible front-tooth gap, the patient reported feeling self-conscious for years, frequently hiding their smile in social and professional settings.

Upon initial clinical evaluation and radiographic analysis (CBCT), pronounced alveolar bone resorption was observed in the missing tooth area due to long-term disuse. Compounding the challenge, the adjacent front tooth root had fractured; while remaining fully shrouded within the soft gum tissue, this compromised root posed a latent infection risk and further undermined the structural integrity of the anterior maxilla.

To resolve both severe localized bone deficiency and psychological distress without subjecting the patient to multiple invasive surgeries, Dr. Tang’s team devised a specialized protocol: atraumatic extraction of the fractured root, immediate single-implant placement with a cantilever bridge framework, and targeted bone grafting.

The primary surgical challenge was to manage severe bone loss at Tooth #2 while also addressing the compromised adjacent tooth root. Placing multiple implants into areas with varying bone density was neither cost-effective nor clinically sound.

• Management of the Fractured Root: During the same surgical session, an atraumatic extraction of the fractured adjacent root was performed to eliminate future infection risks while fully preserving the surrounding gingival socket.
• Strategic Implant Placement: Instead of attempting to anchor directly into the severely resorbed bone at the Tooth #2 site, the surgical team anchored a single high-stability implant into the denser bone of the adjacent anterior maxillary zone, achieving optimal primary stability.
• Hard & Soft Tissue Contour Preservation: High-grade bone grafting material was carefully applied to the bone-deficient regions around Tooth #2 and the extraction socket. This approach reinforced the basal bone architecture, effectively prevented loss of facial volume, and maintained a natural, harmonious gumline contour to support the forthcoming provisional restoration.
• Biomechanical Restorative Design: A customized two-unit cantilever bridge framework was securely fastened onto the single implant post, replacing both missing teeth while avoiding the need for a second surgical site.

By avoiding an extra implant placement, the treatment significantly reduced patient trauma, surgical recovery time, and total financial cost.

Immediately following the procedure, a customized provisional (temporary) crown was fitted, instantly re-establishing the patient's natural smile line, upper jaw volume, and basic function. The patient was informed that this provisional crown serves to protect the site and guide gum healing during a 4-to-6-month osseointegration period, after which the final permanent ceramic restoration will be delivered.

The patient was also advised that the provisional crown serves as both a biological seal and an aesthetic placeholder. It may require occasional adjustment or recementation during healing—this is a normal aspect of the immediate-load protocol.

The patient expressed total satisfaction with the immediate aesthetic improvement, feeling confident to smile openly once again.
